Contacts page, email page not working

Having problems with my contacts page. http://www.ionic-calcium.co.uk/contact/index.cgi

My original web host worked fine, so i think BT does not support this or my page paths are wrong.
Can someone tell me why my contacts page does not work?

or can someone tell me what my paths should be?

eg i currently have:

or perhaps someone could give some alternative code for an Email form?
