re:web form

hi I have published my website henrysbarbersnewcastle.co.uk but when you try to submit a form you do not get the thank you page and i do not recieve an email with the details. Please can someone help and explain where i have went wrong.

How did you solve the problem? I have the form on my page and people can fill it in online and I get an email, but the email just shows me a blank form, not what they have filled in!
