Getting a Text Attribute to appear

Hi,

I have my website up and running. I am looking to add some text fields to my website so that when my customers order a certain product (Dog or cat tag) they can add text that they want to appear on the tags. This text is then sent with the order. I can get a drop-down box appearing to select the correct tag but none of the text fields appear on the website.

I have setup a product type with 4 attributes, 1 drop-down box (which appears on website) and 3 text attributes. All 4 are set to visible but as stated earlier the 3 text attributes don't appear. Can anyone help me with this?
