BT 2700 HGV Pinholes firewall rule lost and unassigned

I am having an issue where I frequenty have to re apply the pinhole rules, they remain configured but just somehow become un assigned from the rtelevant destinations/rules.

Here is a copy of the log entry when it appears to have failed:
