PHP page not displaying

I have a form on one of my pages, which links to a PHP file. All of my other pages are displaying but this page just throws up a HTTP 500 error. I have checked the link, and tried it using PHP4 and 5. With version 4, I get a blank page rather than an error,  but thats it - any ideas?

Can you let me know if you find a solution. I'm experiencing the SAME THING. My php contact forms work fine when I test them on my server, but after uploading to my client's GoDaddy hosting account I get that 500 error.

 

I have uploaded in another format using Fetch. The format is Apple SIngle. The pages show up and the forms work, but now there's extra code/garbage characters at the top of each page. They get added to each page on uploading in Apple Single format.

 

The GoDaddy account is a Unix server and I've tried PHP 4 and 5. No difference.
