Uploading Files Via A Form

Hi All

Can anyone suggest how I may include a field within a form so that a visitor may upload a file that is attached to the email that is sent with the form? I am using Easy Site Wizard Pro and also I occasionally edit the HTML code via ESW, so if someone can give me the necessary code to add then maybe that is a solution?

 hi - you need to include some php code within the forms html AND the target directory has to have write permisions set.
