To cut a long story short for the last few quarters my company has done the following
1. Paid our bill by BACS payment using the correct bank details provided on the bill
2. Sent a remittance to the email they ask it to be sent to including the account number and bill ref etc
3. Weeks later receive a letter asking where our payment is and threatening to cut us off
4. Phone BT up, tell them we have paid and eventually receive a letter apologising
5. When the next bill is received repeat the above steps
To stop this happening the last 2 times i have phoned up explaining we have paid i have said we will pay by DD instead. We need two signatories at my company so they have to send us a Direct Debit Instruction to fill out, no problem you would think....except neither time they have actually sent us the Instruction so can't fill it in!!! (don't know why i am surprised, when i spoke to them at the end of March to try and sort our billing problems i was told the last time i had rung was 4/5 months ago when i called in February so i think some of their call operators are having a laugh.)
Does anyone know anywhere i can download the Direct Debit Instruction as i am sick of having to deal with BT?

Thank you for trying to help but this section doesn't solve my problem. When i went through the process of signing up there was a tickbox that said 'tick here if you are the only person that is needed to authorise payments from your bank account' or something very similar. As mentioned in the original post i am not able to do this, Two signatories are needed and thus i need to fill out the Direct Debit instruction form we have never received.
I too, would like to know how you set up a two signatory Direct Debit. Some other utilities have a proform to download- not the BT site as far as I can see.

Strange suggestion - not sure if you can get a proforma DD form from your bank (as though you are intending to set it up for a customer), complete the relevant sections to send to BT to then process accordingly??
